The Pat Mac Fund will extend charitable activities to supplement qualified veterans who live in the Tri-State area during the fiscal year 2014-2015. Veterans may apply for assistance at the beginning of the third quarter in 2014 (April) and every other quarter thereafter. All applications are due Friday, June 27, 2014 by 12:00 AM, decisions for disbursement will be made before the end of the same quarter of which the applicant applies (by 12:00 AM of the first day of the next quarter). If applications are not deemed qualified (criteria highlighted below), a disbursement will not be made in the quarter there of, however, denied applicants may reapply at the beginning of the next quarter unless otherwise notified.

Applicants must complete/provide the following information for The Pat Mac Fund Assistance Program:

·         Complete The Pat Mac Fund Assistance Application in full.

·         Specify the branch of service, rank, unit, city, state, zip code, date of enlistment, and date of separation

·         Provide a list of military awards or achievements, if any

·         Provide 2 references

·         Provide a written or typed description of hardship or circumstance requiring assistance

·         Provide a copy of form DD-214 with application

·         Provide a valid scanned state or government I.D. (front and back)

·         Sign, date, and send documents to the indicated address on assistance form


Applicants must meet the following criteria to be eligible for The Pat Mac Fund Assistance Program:

·         Previous or current military enlistment in the United States Military

·         Currently suffering or previously suffered a personal hardship


The following information, for the purpose of The Pat Mac Fund Assistance application process, defines a “personal hardship”:

·         The individual is suffering from Post Traumatic Stress Disorder

·         The individual is suffering from financial hardship in the form of bankruptcy or loan default

·         The individual is unable to afford basic utility bills (water,gas, electric, heating oil, etc.)

·         The individuals is unable to afford medical payments of any kind


            The Pat Mac Fund reserves the right to deny fund disbursement if the application contains falsified or misleading information, incomplete information, missing information, and/or the application is submitted past the deadline of the current quarter indicated.  The Pat Mac Fund will accept applications until the established budget for the assistance program is exhausted for the current fiscal year.
